Understanding Recurrent Pregnancy Loss

Recurrent pregnancy loss is defined as two or more consecutive pregnancy losses, as per the American Society of Reproductive Medicine (ASRM). A pregnancy loss is clinically recognized when it ends involuntarily before 20 weeks and has been confirmed via ultrasound or tissue identification.

Diagnosis and Testing

The first step in addressing recurrent pregnancy loss is a thorough medical, surgical, family, and genetic history assessment, followed by a physical examination. Karyotype analysis of both partners may be conducted to identify any chromosomal abnormalities that could contribute to miscarriage. However, this test is performed selectively, as such abnormalities are relatively rare.

Evaluating the Uterus

Evaluating the uterus and uterine cavity is crucial in diagnosing recurrent pregnancy loss. Several methods include:

  • Ultrasound: Often the first test to assess the shape of the uterus and detect fibroids.
  • Saline Ultrasound: Fluid is injected into the uterus to check for abnormalities such as polyps, fibroids, or scarring.
  • Hysterosalpingogram X-ray: An X-ray of the uterus and tubes to determine if they are open and functioning correctly.
  • MRI (Magnetic Resonance Imaging): Used for detailed evaluation of fibroid locations and uterine shape abnormalities.
  • Hysteroscopy: A minor surgical procedure involving a camera inserted through the cervix to visualize and treat abnormalities within the uterus.

Blood Tests and Antibody Screening

Antiphospholipid antibodies, including anticardiolipin antibody and lupus anticoagulant, are often checked, as these are associated with antiphospholipid syndrome, which can lead to pregnancy loss. Screening for inherited thrombophilias is not routine unless there is a personal or family history of abnormal blood clotting.
